Program Information Form

The Program Information Form (PIF) is used to collect out-of-school time programmatic data. Programs, communities, and stakeholders can utilize the information collected to better visualize demographics, view programmatic offerings, and identify strengths and weaknesses. The Program Information Form is not the end-of-year report for either DWS, OCC, or USBE funded programs. Each of these state agencies will contact you with information on how to submit those. Fill Out the Form What information is collected? What information is collected? The Program Information Form provides UAN with vital information on the overall climate and makeup of afterschool programs throughout the state of Utah. We do this through collecting information on location, youth and staff demographics, program operations, funding sources, and partnerships. This information may be used for afterschool advocacy and education efforts, State of Afterschool reports, annual State of Afterschool - Year At A Glance reports, and to update our Find A Program Map. The Utah State Board of Education, Department of Workforce Services – Office of Childcare, and Utah Education Policy Center may also use the information you provide in this Form. How Do I Fill Out the Form? How Do I Fill Out the Form? Download the Fillable Program Information Form (PIF) PDF by clicking on the, "Fill Out the Form" orange button above. Collect the requested programmatic information on this document. This will greatly speed up the digital submission process and help avoid potential loss of data. Once you've filled in the form you're ready to move on to the next step, "Submitting Your Program Information Form." Where Do I Submit the Form? Where Do I Submit the Form? Only use one computer and browser when filling out the submission form to avoid losing data. To access the submission link, click the orange button above, "Submit the Form." At the beginning, you will need to enter in basic program information including a program ID number. If you do not remember it or do not have one, please contact UAN. After filling out program data, the majority of information can be copied and pasted from the PIF PDF to the Submission Form. Most digital submissions can be entered in about 20 minutes (per site). The FY25 Program Information Form submission link will open in the spring and submissions are due by May 31, 2025.
